Request for Internet, Telephone and TV Service

Purpose: To provide information pertaining to requirements for Telecommunications Services on Army Garrisons

Coordination: Installation Director of Morale Welfare and Recreation (DMWR) will populate this requirement worksheet soliciting input from the following agencies. Upon completion, the worksheet will be provided to the FMWRC TOM for further action.

STOP! The following information is required to complete this form:

  • Contact Information for the Requestor (Organization Name, Person's Name, Phone, Email, Mailing Address)
  • Garrison Name (where the service will be located)
  • Is the requirement new?
  • Will the service be free to the end user? (If the service will be free to the end user you will be asked for content filtering requirments.)
  • Is this a short or long term agreement?
  • Expected length of service
  • Funding source
  • Building Information for at least one building. (Building Type, Building Number/Name, Service Requried, Service Start Date)

Key personnel to secure the required information to complete this document include:

  • Garrison Command
  • Installation Director of Public Works (DPW)
  • Network Enterprise Center (NEC)
  • AAFES General Manager (GM)
  • FMWRC Telecommunications Operations Manager (TOM)

Please review and comment on the request to provide telecommunications services on the following screens. Comments should include but not be limited to impact of current or future projects, known troop movements, and other factors impacting deployment of these services.
